Under the direction of the Program Manager of HIV/STI Services, this position is accountable for assisting Portuguese-speaking clients living with HIV/AIDS in obtaining medical, housing, social and legal services and facilitating access to counseling services.
Essential Duties and Responsibilities:
1. Performs screening, assessments, intakes referral and follow-up.
2. Develop and monitor individual service plans (ISP).
3. Follows the progress of clients to help coordinate services as their needs change.
4. Complete a reassessment with each client every six-months
5. Maintain regular phone and in-person contact with clients based on BPHC Non-Medical Case Management Standards of Care guideline while keeping the notes in each file up to date.
6. Provide crisis intervention, support and advocacy services
7. Accompany clients, when appropriate, to medical and other appointments
8. Provide supported referrals to medical care, substance abuse, mental health and other services as needed.
9. Assist clients with applying for rental assistance and searching for housing and provide follow-up to ensure stability.
10. Stay current with HIV/AIDS trends, services, and treatments
11. Attend individual supervision and staff meetings as scheduled
12. Attend all trainings mandated by Boston Public Health Commission and those deemed appropriate by supervisor.
13. Submit all the reports on time
14. Work with community agencies in liaison capacity. Cultivate and maintain positive working relationships in the field.
15. Required to participate in organizational events
16. Perform other duties that may be required
Qualifications:
Education: BA degree in related field preferred
Experience: Extensive knowledge and experience in HIV/AIDS.
Special Skills: Strong oral and communication skills required, Bilingual/Bicultural in Portuguese/English and knowledge of the Portuguese speaking communities.
Strong organizational skills desired and ability to work both independently and in a group setting.
Starting annual salary range commensurable with experience: $52.000-$58.000
